EMPLOYEE BENEFIT OVERVIEW
WRHS provides an excellent benefit package to meet the needs of our diverse workforce. We recognize that benefits are a major factor in our total compensation plan. To give you an idea of how we take care of our employees, we have provided a brief summary of benefits offered to those employees who work 20 hours or more in a benefit eligible position. Questions about specific plan information can be directed to the Human Resources Office.

HEALTH INSURANCE
| Employees have the opportunity to choose from two plans Wellness and Standard. Premiums are shared with employees and deducted pre-tax through payroll deduction. Employees working in benefits eligbile positions of 20 hours or greater are eligible to participate and cover elgible dependents after 31 days from hire.

PRESCRIPTION DRUG PLAN
| Premium is included in Health Insurance Premium. Along with discounted rates on prescription drugs, a mail order option is available for maintenance prescriptions (90 day supply). Coverage is available to employees and eligible dependents 31 days after hire.

DENTAL INSURANCE
| Covers 100% preventative services, 80% basic and 50% major services with a $50 deductible per insured. Employees have no restrictions on dental providers. Premiums are shared with employees and deducted pre-tax through payroll deduction. Coverage is available to employees and eligible dedpndents 31 days after hire.

VISION INSURANCE
| Covers 100% of eyes exam, frame and lenses when an In Network provider is selected - optional contact choice is available. Benefit available every 12 months to employee and eligilbe dependents. Coverage is available 31 days after hire.

LIFE INSURANCE:

WRHS pays the premium for one times annual salary up to $50,000.00 for all Full-time (32 hours a week) employees. Employees have the option to purchase additional Supplemental Life Insurance up to three times annual salary (up to $450,000.00 maximum). Benefit is available 31 days after hire. $5000 Dependent Life Insurance is also available for eligible dependents.

LONG TERM DISABILITY INSURANCE

Ensures continuation of pay when you cannot work due to a long-term disability. Benefit amount is 60% of your monthly base salary. Employees working positions of 32 hours or greater are eligible to participate. Benefit begins after one year of full time employment

RETIREMENT SAVINGS PLAN
| A portion of employee's paycheck can be transferred into a tax-sheltered annuity for retirement savings. Employee contributions are pre-tax up to the IRS limitations. After one (1) year of employment WRHS will make contributions equal to 2% of base salary into an acount and match employee contrbutions up to 3% of base salary. Employees working in benefit eligible positions may participate. Employee contributions my begin upon hire, employer contributions begin after one year of employment.

EMPLOYEE ASSISTANCE PRGRAM |
Available to assit employees and their families deal with the stresses of today's fast paced society and find the solutions to get things back on track. Professional staff provide counseling and follow-up through a confidential program.

PAID ANNUAL LEAVE
| Provides employees with an accrued bank of days to be used for vacation, holiday, personal, and sick leave. Full-time employees (40 hours a week) receive 24 days a year. After 3 years of employment full-time employees receive an additional 3 days and after 10 years of employment full-time employees receive an additional 5 days. Employees who work less than 40 hours a week will receive PAL hours on a pro-rated basis.
