Fees and Insurance
Walton Outpatient Therapy Center services are typically covered by most major insurance plans, as well as Workers’ Compensation, Medicaid and Medicare. However, it is always a plus to check services, covered by your insurance. Also, a script from your physician is required in order for you to receive therapy.

After you have your appointment scheduled, you are asked to fill out several forms, which are attached here for your convenience. Should you choose to print the forms and complete them before your visit, be sure to bring them to your appointment.
